Medical Transcriptionists need to be familiar with hundreds of abbreviations of medical terms.  Doctors use abbreviations to keep their dictations succinct and brief, and expect the transcriptionist to insert the proper expansion.  Even if your client does not want the abbreviation expanded in the report, it is the transcriptionist's responsibility to make sure that the abbreviation is used in the proper context, and that the appropriate spelling of the acronym or abbreviation is used.
